(1) Walk-in (Mon-Fri 8am-6pm), OR

(2) Call for an appointment

Optional Steps:

(1)   Visit website AND

       (a) Schedule appointment & pay for AHCA/DOH Level 2 Screening

       (b) Schedule appointment & pay for ALL OTHER Level 2 Background Checks, OR

(2)    Visit website AND

       (a) Pay online for AHCA/DOH Level 2 Background Checks

       (b) Pay online for ALL Level 2 Background Checks (Except AHCA/DOH)

(3)     PRINT Receipt Form - to be returned to employer after screening.

(4)     PRINT Fingerprinting Applicant Registration & have your employer assist you, OR

(6)    You may pay by credit/debit cards, money order, check, or cash
